Case Management Referrals

A health problem, work related or not, can affect an employees work performance and the health and safety of those who work around them. If you suspect an employee’s performance is suffering due to health, or the health and safety of colleagues is affected by their health then a referral to Wellness International Ltd should be considered.

A referral to Wellness International can help managers and HR understand more about;

  • The health problem and its effects on the employee
  • The likely implications on their work
  • The health and safety of employee and colleagues who work around them
  • Any rehabilitation that may be required and the length/duration of rehabilitation
  • If they are fit for work or if they require a phased return or dismissal on the grounds of ill health.

The Process

Management referrals to Wellness International must be made with the full knowledge of the employee concerned. This is gained via informed consent. This means that the employee must know why they are being referred and what is involved.

A referral form is completed detailing the full history of the problem and any relevant previous episodes of sickness absence. This also details the objective of the referral and any questions that the employer may want answers to, such as “how long are they likely to be on sick leave?”

Once the referral form is received an appointment with the employee will be scheduled. Depending on the complexity of the case the employee may be seen by a specialist nurse or a consultant physician (occupational health doctor).

If an employee cannot attend the appointment because of poor health, the manager must speak to HR about obtaining the consent for Wellness International to obtain a medical report from the employee’s GP or Hospital. Any such request should be made in line with the requirements of the Access to Medical Reports Act 1988. 


Advice given to the manager will not normally contain confidential medical detail, but is concerned with matters of employment and fitness and may include:

  • The identification of a health problem that may impact on an individual’s work.
  • The potential effects of the problem on current and future performance or attendance.
  • Adjustments to the work place or tasks that would assist in maintaining health and reducing the adverse effects of the health problem on attendance and performance.
  • Whether the advised adjustments are temporary or permanent.
  • Timescales for expected improvement.
  • Opinion on the implications of the Disability Discrimination Act 2005.
  • The need for further investigation or medical report from treating doctor
  • The potential for an individual to maintain a good attendance record, and if appropriate, suitability for Ill Health Retirement
  • Proposals for case management or rehabilitation programme where appropriate.

Service Standards

Once the referral form is received it is acknowledged within 1 hour (during working hours 08:30 – 17:00) and the employee is contacted with 24 hours to book an appointment. Following the appointment the report is produced within 48 hours.
